Dr. Ethel L. Escultura is a highly qualified orthodontist with extensive education, professional registration, teaching, work experience, publications, conference presentations, and training.  She holds a Master of Science in Dentistry (Orthodontics) and a Doctor of Dental Medicine from the University of the Philippines Manila.  She is an active member of the Association of Philippine Orthodontists. 

Dr. Escultura has been a senior lecturer and clinical instructor at the University of the Philippines Manila since 2011 and previously taught at Emilio Aguinaldo College School of Dentistry as an Associate Professor. Her work experience includes being a dental practitioner for more than 20 years, an orthodontic specialist, product manager, and an international speaker for EF Line Appliances from Orthoplus. 

She has published research on orthodontic treatments and has presented at numerous national and international conferences. Dr. Escultura has also conducted various workshops and training sessions on orthodontic techniques and functional education therapy across Asia and other regions. She has attended multiple international training programs to further her expertise in orthodontics and Orofacial Functional Education.

兒童時期的牙齒萌發障礙

The disturbance of tooth eruption during Childhood needs to cover that of permanent teeth as well as eruption of primary teeth. There are many variations based on the timing of onset, the timing of detection, the site and the kind of tooth. One of the goals in pediatric dentistry is to achieve a healthy permanent occlusion. In many cases, the eruption disturbance may be successfully corrected by the initial treatment plan. But it is important to consider next steps if the treatment cannot achieve the anticipated results. When we treat the eruption disturbances in children, we need to have comprehensive oral management skills that can address a variety of oral troubles. In this lecture, I will discuss how to manage eruption disturbances by reviewing difficult cases and will explain the small insights and comprehensive oral management that only pediatric dentists can provide.

運用預製功能裝置來進行全方位的咬合管理：

從乳齒列開始

  Malocclusions in the primary dentition are common, with anterior crossbite and deep overbite being the most frequently observed conditions. Studies have reported that increased overjet, deep bite, and posterior crossbite are prevalent among young children, with variations influenced by genetic and environmental factors. Although some mild cases may self-correct with growth, persistent malocclusions can lead to functional imbalances and increased skeletal discrepancies. If left untreated, deep overbites may worsen as the permanent dentition emerges, and anterior crossbites may hinder normal maxillary development. Therefore, early diagnosis and intervention are crucial to preventing long-term complications.

  The formation of occlusion during the transition from the primary to the mixed dentition is influenced by skeletal growth, dentition changes, and functional adaptation. Oral muscles, including the tongue, lips, and buccal musculature, play a vital role in maintaining dental arch integrity and occlusal balance. Any imbalance in muscle function may disrupt occlusal development, leading to worsening malocclusion. Particularly, children with severe overbite and mouth-breathing tendencies may exhibit altered lip positioning and mandibular growth patterns, increasing the likelihood of Class II malocclusion. Conversely, in cases of anterior crossbite, inadequate maxillary incisor eruption may result in persistent skeletal discrepancies.

  Prefabricated functional appliances offer a non-invasive approach to correcting these issues by guiding normal muscle function and occlusal development. These appliances help reposition the tongue, stabilize oral musculature, and promote balanced skeletal growth. By preventing excessive vertical eruption of anterior teeth and encouraging mandibular advancement, they contribute to improved occlusal relationships. However, as these appliances are prefabricated, individual adaptation may vary, necessitating careful patient selection and monitoring.

  This lecture will explore clinical cases demonstrating the application of myofunctional appliances in primary dentition malocclusions. By analyzing case studies, we will discuss key strategies for enhancing appliance effectiveness, optimizing patient compliance, and ensuring long-term occlusal stability. Through early intervention and targeted functional therapy, pediatric dentists can play a pivotal role in guiding craniofacial growth and improving occlusal outcomes in young children.

非活性乳牙根管治療的預後挑戰—從失敗談起

主題聚焦於乳牙牙髓治療失敗可能的後果及因應對策。乳牙牙髓治療的成功率一般有80%左右，在正確診斷牙髓問題後，需要權衡選擇最保守、最高成功率的治療方式，同時要減少後續併發症的機會。因為失敗可能會對恆齒有不良影響，術後需要定期回診，並對異常狀況做適當處置。在本次演講，特別針對乳牙牙髓治療失敗的臨床案例處理與預防做經驗分享和討論。

未成熟恆齒牙髓病治療的考量與挑戰

學齡前後的孩童牙齒常常會受到蛀牙、外傷或是發育異常所造成的感染威脅而影響未成熟恆牙的發育。因此未成熟恆齒的牙髓病治療與孩童口腔健康的一生息息相關，近年來活髓治療（Vital pulp therapy）與牙髓再生治療（Regenerative endodontic procedures）等術式被廣泛運用於這類牙齒的治療，希望能藉由這次分享探討更多治療上的可能性拯救更多孩童的牙齒。

Tough Talk in Pediatric Dentistry

Previous studies report that genetic and environmental elements influence maxillofacial growth and that improper breathing, chewing, sucking, and swallowing are promoting factors of malocclusion. The clinical presence of anterior open bite, skeletal class II or class III, is often associated with orofacial dysfunctions. Emerging evidence supports the important role of abnormal swallowing in the relapse of orthodontic treatment. 
Developmental physiology distinguishes infantile, mature, and inconstant swallowing. Infantile or visceral swallowing is characterized by a forward movement of the tongue which promotes a continuous boost against the lingual surfaces of the anterior teeth. This clinical situation is physiological until 4 years, while afterward, it is considered dysfunctional and usually associated with malocclusions. On the other hand, a mature or somatic swallowing pattern is characterized by a cranial movement of the tongue and the resulting boost is addressed on the incisive papilla. The maturation from a visceral to a somatic deglutition is gradual, and the transitional period is defined as inconstant swallowing.
Swallowing patterns have been linked to environmental factors, tongue position and thrust, mouth breathing, and nonnutritive oral habits. The presence of altered swallowing can also be attributed to ankyloglossia, or tongue-tie: a congenital oral anomaly characterized by an abnormally short lingual frenulum that negatively affects the growth and development of the stomatognathic system.
Within a dysfunctional swallowing pattern, close attention must be paid to the tongue rest position. Proffit et al. measured the force levels of the tongue against the upper incisors and palate during rest position and normal swallowing. They stated that the tongue resting point was a more important contributing factor than the swallowing position in developing dental arch form. The results from the cinematic-magnetic resonance imaging analysis described horizontal tongue movement among these individuals and the tendency to place the tongue between teeth while speaking and swallowing. Therefore, in clinical practice, dentists must consider many aspects, such as breathing, tongue resting position, swallowing, pronunciation, chewing, etc.

Impacted Teeth Management

阻生齒的治療是困難且複雜的，臨床上我們可藉由CBCT的幫助，讓我們更明確的了解阻生齒與鄰牙的相對位置、是否造成鄰牙牙根吸收及強迫萌出的路徑是否有障礙，也能提供資訊幫助於我們設計更精確的biomechanism。但即便如此，治療時間仍是冗長而繁瑣，且充滿不確定性，因此治療計畫選擇拔除阻生齒或強迫萌出的選擇變得相當煎熬。本次藉著病例分享來尋找治療上所遇到的困難以及治療上應注意的事項，以期減少併發症的發生，讓治療能順利完成。
